Help IDENTIFY this dying coral frag

Got this frag a few weeks ago, along with a few other frags. I am not sure what the name of this one is (image below), but it seems to be dying. The other corals are doing well. I have tried moving it around to get different air flow and light, but it is not helping. Nitrites and nitrates are at 0. Salinity is at 1.023, temp at 78degrees, and pH at 8.2.
 
What is the name of this coral and what are the preferred parameters / water flow?

When I first got it, the tiny polyps would protrude slightly from the holes and were very bubbly/ flowery looking..

Goniopora maybe? Hard to tell for sure from that pic.
