Anyone got Greenex directions????

Could someone tell me what the directions for Greenex medication are.

 

on the bottle it says:

skake well...one drop per gallon, and then says for directions on exact prodcut use, refer to the back of the box,  and of course, i've lost the box.

 

So if you have the box, and wouldn't mind, could you tell me what the directions are for the "exact product usage"

p.s. has anyone had any luck using this medication to get rid of ich in a reef tank?

its not good to add ANY chemichal in a reef, what they say and what it does is two diffrent things .if  your water is up to par you wont have any problems with ich.if you have ich on a fish take it out to a hospital tank and treat it in there.....i would never add any of that chem garbage,who knows what effect it has on the organisms in your tank.a fish with ich may also clear up in a healthy reef,also feed with garlic extracts a good ich inhibitor.
