Damsel died randomly?

This is my first post on here. This is my question, has anybody had a damsel randomly die despite great water quality? It is a 30 gallon that had been cycled 2 weeks before I added the damsel. I test every other day and my water parameters are normal.

Ammonia 0

Nitrite 0

Nitrate ~5

The only thing different is I did the first water change that he was in the tank for. Anybody had something similar to this happen or anybody have any idea why he died? Thanks in advice.
