Worm or parasite? ID

I posted this on RC but you know how they are...with the actinic light on only the other day I saw a worm chillin on the glass near some algae, it was really small and white/clear, about 1/8th inch long but the weird thing was it had 2 antanea(sp?) sticking out of its head that were as long as its body, anyone know what this is?  It reminded me of freshwater gill/skin flukes but I don't have anything in my tank it could feed off of and the tanks been set up for like 4-5 months now....i also saw this jellyfish lookin things, basically a really small bubble with tiny tentacles floating around in the current...maybe hydroid jellys but with my luck probably aiptapsia offspring lol

Hey famous dude!

The worm is prolly a chiton

The Jellyfish are just that Jelly fish....They seem to come out every once in a while in my tanks, I have witnessed zooanthid polyps catch and eat them.
