Toadstool coral has holes. Help!

Hi Friends!

 

My Toadstool has a big hole in it. See images. You can see right through it.

 

Aquarium parameters look normal. I caught a hermit clinched to it this morning in a different spot. Are they the culprit or is it splitting.. or dying?

I had that happen once.....some type of worm ate from the bottom straight up the center and out the top......had a little sand-like tunnel running from the sand to the bottom of the toad.

There is a worm that can do this. There is also a nudibranch.
