I thought I would post this scenario in the format of a medical intern presenting a case to his registrar. Hope you find it interesting and please share any provisional diagnosis' you can think of! Patient: Bob, 18 month old, male Presenting Complaint: Dead History (Hx) of Presenting Complaint: - 2 day Hx of mild grey tinge to slime coat and occasional scratching against live rock - No signs of floating sideways - No other physical signs of sickness - Deteriorated rapidly this morning Past Medical Hx: - None Prescriptions: - Recent switch to double dose of Phosguard after heavy dosing of Lanthanum Salts and darkness was ineffective in killing Bryopsis algae. Forgot to rinse last dose of phosguard. Social Hx: - Spent past month living in 6 week old Nuvo 8 Gallon tank Investigations: Last testing 2 days ago - Ammonia 0-0.25ppm - pH: 8.0 (added buffer to correct on Sunday) - Salinity: 1.0243 - kH: 7 - Temperature 25’C - All other tests negative - New testing shows pH is up to 8.2, kH up to 8 and ammonia down to 0 Family Hx: - Larger, moderately aggressive clown added 1 wk ago - A goldfish died in tank in same room within hours.
