I suspect something like a nudibranch is eating it, judging by the placement and the size of the hole. I would take a turkey baster and see what comes out. If alot of detritus comes out move it to an area of higher flow and keep an eye out for nudibranchs, that are cream, tan or white in color, they like to eat toad stools. Good Luck.

I tried Lugols on my toad stool and still lost it, but I think it had a secondary bacterial infection too. Sorry not much help I know. Nudibranch and sea slugs are basically different animals but most people use them interchangeably, so if you are look on the net try sea slugs as well.
Did trying the turkey baster turn up anything, because if you get alot of detritus and tissue your problem could be bacterial.
I would try Revive before Lugols, but that is just my experience.

If it's in one area you could just cut it off and put the leather somewhere else, or frag it

Mine went through something similar a few months ago. I cut the crown off just above the affected area and placed the two pieces away from each other. The base continued to wither away, faster than before, but the crown healed. I am still unsure what the cause was, but cutting it helped mine survive.
